To Make a Report

You may use either of the following two methods to submit a report:

This process is not intended to replace or supersede normal managerial and Human Resources communication channels.

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

EthicsPoint is NOT a 911 or Emergency Service: Do not use this site to report events presenting an immediate threat to life or property. Reports submitted through this service may not receive an immediate response. If you require emergency assistance, please contact your local authorities.

It is essential that we maintain the complete confidence of our volunteers, employees and donors, so business transactions must be handled in a way that does not result in adverse criticism from the public or in any way damage the Food Bank’s reputation as a responsible non-profit agency.

Second Harvest of Silicon Valley expects its volunteers, agencies, and employees to use good judgment, to apply the highest ethical standards and to refrain from any form of illegal, dishonest, or unethical behavior.

Reports made to EthicsPoint and Ethics Hotline will remain confidential and anonymous. Any employee, volunteer, customer, vendor, agent or invitee may contact Ethics Point and the Ethics Hotline, in good faith, to report violations of any of the policies in the Employee Manual or any concern regarding the operation of Second Harvest of Silicon Valley and services to its agencies or the public. The individual need not identify him/herself and his/her communication will be kept confidential.

Reporting of Violations or Suspected Violations

Should you suspect fraud, abuse or misuse of Second Harvest of Silicon Valley resources or assets; encounter dishonest actions or deeds; suspect conflict of interest; experience or become aware of any kind of behavior that violates Second Harvest of Silicon Valley’ policies, or local laws and regulations, you have a responsibility to report the violation or suspected violation to the appropriate entities within Second Harvest of Silicon Valley

Individuals who do not feel comfortable reporting violations of this Policy to the persons listed above may also call the Ethics Hotline. This can be done anonymously. The subject matter content will be kept confidential to the extent possible. The telephone number for the Ethics Hotline is 1-888-260-5912 or www.ethicspoint.com.

All reports involving a violation are treated seriously and will be fully investigated to determine the facts and resolution appropriate under the circumstances. Second Harvest of Silicon Valley expects any report of a violation will be made in good faith and is a real and legitimate concern that the employee believes should be addressed. Second Harvest of Silicon Valley will not tolerate retaliation in any form against individuals who make complaints in good faith.

Discipline for Violations

Violations of the Code of Conduct Policy will not be tolerated. Standard of performance and Conduct violations, depending upon the severity, may subject the employee and others to disciplinary action up to and including termination of employment or other relationship, as well as to the possibility of referral for possible criminal or civil legal action.
